School, child care and children's camp facilities are responsible for reporting suspected and confirmed COVID-19 cases.
This reporting form must be immediately completed by an administrator or support staff when COVID-19 is suspected or confirmed in a staff member or student at your facility. All information collected by this secure form will be used to support case management and contact tracing by Public Health.
To report suspected cases, call the COVID-19 Info-Line. A suspected case is someone who has COVID-19 symptoms AND an exposure. An exposure is:
Don't report staff or children who are symptomatic and going for testing without any known exposure even if a serious occurrence report is completed.
Don't report suspect or confirmed COVID-19 cases who are staff / students who have only been working / attending school virtually or have not presented in person at your facility; these cases will be followed up through the regular COVID-19 case and contact tracing process.
