Certificate of Insurance
The Niagara Region Certificate of Insurance form is now available on line.
Individuals and organizations who supply goods and services to The Niagara Region, Niagara Regional Police Services Board, Niagara Regional Housing and/or Niagara Economic Development Corporation are required to provide evidence of insurance as part of the terms and conditions of a contract, purchase order, permit, or other form of agreement.
The Niagara Region has three standard forms. The appropriate form is to be completed only by your insurer or their authorized representative. These forms are the only Certificates of Insurance accepted by The Regional Municipality of Niagara. Once completed, please forward the original to the requestor.
- Standard Certificate of Insurance This form is for agreements, contracts, permits, purchase orders or other forms of agreement whereby proof of liability insurance is required. - also available as a Word version
- Contractor Certificate of Insurance This form is for infrastructure design and construction projects which requires not only proof of liability insurance but other types of insurance as part of the terms and conditions of a contract or other form of agreement. - also available as a Word version
- Professional Certificate of Insurance This form is for consultants and other professionals who provide services for or on behalf of The Niagara Region, Niagara Regional Police Services Board, Niagara Regional Housing and/or Niagara Economic Development Corporation. - also available as a Word version
